How to Report a Concern
You can report anything you believe, in good faith, is a violation of the law, UNT System Regents Rule or the policies of the UNT System Administration. There is a list of incident types from which you can choose or you can describe the incident in your own words. Types of incidents include:

- Environmental Health and Safety
- Ethics
- Financial Reporting
- Financial Services, P-Cards, System Marketplace
- Nepotism

“Good faith” means the person who is making the report reasonably believes, based upon training and experience, that the concern about the decision or conduct they are reporting does not comply with the law, UNT System Regents Rule or the policies of the UNT System Administration.
Any employee, member of the public, or third party vendor (contractor) who observes or finds themselves in a situation that they believe, in good faith, may raise concerns about unethical, unlawful, or other noncompliant decision-making or conduct may make a report.
It is your preference whether to use the website or phone. A OneTrust representative will assist you with making a report whichever option you choose. Either way, your concern will be anonymous (if you choose) and confidential.
What to Expect in the Reporting Process

For OneTrust (the Trust Line vendor) to send your report to the System Administration Compliance and Ethics Program.

For your report to be handled according to System Administration internal procedures, including an investigation when appropriate.

To be notified how your concern is resolved.

To not experience retaliation for reporting your concern in good faith.
OneTrust is a private company that offers a way for individuals to confidentially report suspected wrongdoing and other non-compliance in furtherance of the UNT System's commitment to ethical, honest, and lawful decision-making and conduct.
Your incident report is NOT given to law enforcement unless it is required by law or the appropriate UNT System Administration official(s) believes it potentially involves a criminal matter.
In all instances, every effort will be made to protect your identity and information that possibly could reveal your identity. Below are the different levels of anonymity you may select when you make your report:

Remain completely anonymous: You do not reveal your name or contact information.

Remain anonymous toward the organization: You are comfortable revealing your name and contact information to OneTrust, but not to the UNT System Administration. (OneTrust may contact you confidentially to gather additional information about the report but will not reveal your identity at any time to the UNT System Administration).

Share my name and information: You choose to reveal your name and identity to both OneTrust and the UNT System Administration.
All reports received by OneTrust are routed to specific individuals within the UNT System Administration and OneTrust's legal counsel who are responsible for reviewing the information you provide. Each individual has extensive experience reviewing reports, conducting investigations, and resolving concerns in a thorough, impartial, and confidential manner.
Retaliation against anyone for reporting suspected wrongdoing or noncompliance, or against anyone participating in an investigation related to a report is strictly prohibited. Anyone found to have engaged in retaliation is subject to disciplinary action, including termination of employment or business relationship. If at any time you believe you have been subject to retaliation after raising a concern or participating in the investigation related to your report, please immediately report it to OneTrust or to the UNT System Administration Compliance & Ethics Program.
Following Up on a Concern
You'll be assigned a number to access your report and given the opportunity to create a personal password, which will allow you to check the status of your report. You will not be able to change any information you provided in the original report, but you can add additional details using the private case message board that is automatically created with a report.
